Prophylactic Irradiation of the Lung

چکیده

Situs inversus totalis is a rare congenital anomaly, as demonstrated by mass radiographic screening in Norway which yielded an incidence of 1 in 10,000.@ Single coronary artery is also rare; a recent review suggested an incidence of 4 in 10,000.' The chance of these two lesions occurring simultaneously is thus about 4 in 1 x 108. Hypertrophic subaortic stenosis is uncommonly as sociated with other cardiac anomalies.° The constella tion of defects in the above case is therefore unique. In this patient, the unusually mild, longterm, clinical course is attributed to infundibular stenosis resulting in a natural “¿ banding― of the pulmonary outflow tract. This is analogous to surgical banding of the pulmonary artery in order to prevent massive pulmonary blood flow and pulmonary hypertension in neonates deemed too young for closure of a ventricular septal defect.7 The weakness, chest pain, and syncope which devel oped in our patient at age 35 are attributed to the combination of left ventricular outflow obstruction and ventricular septal defect rather than to any effects of the single coronary artery which was widely patent anglo graphically. Complete surgical correction of the defects resulted in marked improvement in the hemodynamic and clinical states. The development of aortic regurgitation after surgery

Prophylactic cranial irradiation in small-cell lung cancer: is it ever indicated?

Prophylactic cranial irradiation (PCI) is being reintroduced into multimodality treatment protocols of patients with small-cell lung cancer (SCLC). The history of its use brings interesting insights into clinical evaluations of treatment strategies and design of relevant and informative trials. The critical issues of effectiveness and overall health gains of prophylactic cranial irradiation hav...

Prophylactic cranial irradiation for patients with small-cell lung cancer in complete remission. Prophylactic Cranial Irradiation Overview Collaborative Group.

BACKGROUND Prophylactic cranial irradiation reduces the incidence of brain metastasis in patients with small-cell lung cancer. Whether this treatment, when given to patients in complete remission, improves survival is not known. We performed a meta-analysis to determine whether prophylactic cranial irradiation prolongs survival. METHODS We analyzed individual data on 987 patients with small-c...

High Grade Glioma after Prophylactic Cranial Irradiation for Small Cell Lung Cancer

We report a patient with an initial diagnosis of small cell lung cancer treated with prophylactic cranial irradiation (PCI), who subsequently developed a second primary brain neoplasm within the PCI volume. Pathology review was consistent with astrocytoma, WHO grade IV (Glioblastoma Multiforme). After describing our case, we review the literature on the development of a second brain primary aft...

SCLC extensive disease – treatment guidance by extent or/and biology of response?

In extensive disease of small cell lung cancer a doubling of the one-year-survival rate was reported in August 2007 by prophylactic cranial irradiation applied to patients who experienced any response to initial chemotherapy. We discuss the treatment concept of extensive disease in the face of the latest results and older studies with additional thoracic irradiation in this subgroup. A randomiz...

Circulating tumor cells correlate with recurrence in stage III small-cell lung cancer after systemic chemoradiotherapy and prophylactic cranial irradiation.

OBJECTIVE We investigated the correlation between circulating tumor cells and the incidence of brain metastases as a first site of recurrence among patients with small-cell lung cancer after systemic chemoradiotherapy and prophylactic cranial irradiation. In addition, we assessed the contribution of circulating tumor cells for planning the appropriate total dose of prophylactic cranial irradiat...

Radiotherapy for small-cell lung cancer.

The role of radiation therapy in the primary management of small cell lung cancer is very much a matter of current debate. Its value in palliative treatment is unquestioned. Disappointment in the apparent inability to demonstrate improvement in survival in some randomized studies as a result of locoregional radiotherapy and prophylactic cranial irradiation may be due to the use of inappropriate...
